Mi experiencia como interno en el departamento de Traumatología en el hospital universitario Dr. Luis Razetti

Hoy contare una experiencia particular, una que me enseño que cada elemento a tu alrededor, junto a tu conocimiento pueden ayudarte a salvar vidas.

Uno de los grandes privilegios que te otorga llegar al final de la carrera de medicina es sin duda comprender de primera mano la importancia del conocimiento adquirido durante tu época como estudiante, y comprender que todo lo aprendido en aproximadamente 7 años de carrera debe ser colocado al servicio del paciente para procurar mantener en las condiciones más optimas la salud del paciente.

Y uno de los departamentos más importantes en las emergencias de los hospitales es el de traumatología y ortopedia. Esto debido al gran número de accidentes que ocurren a diario en nuestra ciudad, y que se repite de igual forma en las grandes ciudades de todo el mundo, siendo uno de los más solicitados en los hospitales de gran envergadura.

Un caso excepcional, en donde pudimos observar la calidad de los médicos que forman parte de este postgrado la pude comprender con un paciente, quien al momento de ingresar al hospital, se le observo una laceración en el segundo dedo del pie derecho, dejando ver a simple vista las piezas óseas del dedo, las falanges.

El joven resulto herido por su propia mano, pues según relata el mismo, el corte se lo efectuó al momento de realizar la limpieza de un espacio de su casa, pero no fue una equivocación cualquiera, pues al estar limpiando se encontró con una serpiente que el luego de haberse lacerado el dedo, logro asesinar, no sin antes haberse producido una herida de gran importancia.

El tratamiento inicial estuvo enfocado en evitar posibles infecciones, razón por la cual se indicaron antibióticos de amplio espectro asi como la realización de la limpieza de la herida con material antiséptico, pues estaba sumamente sucia. Luego para mantener en su lugar las falanges, asi como unir parte del hueso que se había separado de la falange distal se decidió utilizar un procedimiento llamado “transfixion ósea”.

Este procedimiento normalmente se haría con un alambre de Kirschner, pero debido a la falta de recursos de los familiares, asi como a la necesidad de un tratamiento inmediato se decidió, mutuo acuerdo del paciente, ejecutar la maniobra con una aguja perteneciente a una jeringa de 20 CC, lo suficientemente gruesa y firme como para mantener las estructuras en su lugar.

Cabe acotar que para poder realizar este procedimiento la analgesia realizada fue algo que no se realiza de manera muy común en los centros hospitalarios. Un bloqueo al nervio poplíteo de la pierna derecha con lidocaína permitió manipular de manera segura y eficiente la herida en cuestión, dándonos el tiempo suficiente para acortar el tiempo que el joven estuvo ingresado en el hospital.

Aunque mi periodo como interno en el departamento de traumatología del hospital Luís Razetti termino antes del primer control del paciente, pude conocer gracias al Dr. tratante, que el joven no perdió el dedo, si no que logro conservar gran parte de la movilidad del mismo.

Quiero agradecerles a todos por permitirme mostrar parte del trabajo que hacemos día a día en los hospitales, para aprender a disminuir las consecuencias de accidentes que pueden marcar la vida de todos en algún momento. Feliz Semana

Today I will tell a particular experience, one that taught me that each element around you, along with your knowledge can help you save lives.

One of the great privileges that reaching the end of your medical degree grants you is, without a doubt, to understand first-hand the importance of the knowledge acquired during your time as a student, and to understand that everything learned in approximately 7 years of career must be placed at the patient service to try to maintain the patient's health in the most optimal conditions.

And one of the most important departments in hospital emergencies is trauma and orthopedics. This is due to the large number of accidents that occur daily in our city, and that is repeated in the same way in large cities around the world, being one of the most requested in large hospitals.

An exceptional case, where we were able to observe the quality of the doctors who are part of this postgraduate course, I was able to understand with a patient, who at the time of entering the hospital, a laceration was observed in the second toe of the right foot, revealing naked eye the bony parts of the finger, the phalanges.

The young man was injured by his own hand, because according to what he says, the cut was made at the time of cleaning a space in his house, but it was not just any mistake, because while cleaning he found a snake that After having lacerated his finger, he managed to kill, but not before having suffered a major injury.

The initial treatment was focused on avoiding possible infections, which is why broad-spectrum antibiotics were indicated, as well as cleaning the wound with antiseptic material, since it was extremely dirty. Then, to keep the phalanges in place, as well as join part of the bone that had been separated from the distal phalanx, it was decided to use a procedure called "bone transfixion".

This procedure would normally be done with a Kirschner wire, but due to the family's lack of resources, as well as the need for immediate treatment, it was decided, by mutual agreement of the patient, to perform the maneuver with a needle belonging to a syringe. 20 CC, thick and firm enough to hold structures in place.

It should be noted that in order to perform this procedure, the analgesia performed was something that is not performed very commonly in hospitals. A block to the popliteal nerve of the right leg with lidocaine allowed us to safely and efficiently manipulate the wound in question, giving us enough time to shorten the time that the young man was admitted to the hospital.

Although my period as an intern in the trauma department of the Luis Razetti hospital ended before the first control of the patient, I was able to learn, thanks to the treating Dr., that the young man did not lose his finger, but managed to preserve a large part of its mobility.
