Aunque en la actualidad, está cobrando cada vez más importancia la coordinación interdepartamental y la actuación multidisciplinaria. Es por ello, que a continuación, les haré un resumen tanto anatómico como también, de la fractura nasal.

    Ante cualquier traumatismo de la parte central de la cara, ha de sospecharse la posibilidad de una fractura nasal. Al margen de las connotaciones legales y de la necesidad de confeccionar un parte de lesiones, una buena anamnesis es imprescindible para orientar el diagnóstico, indagando sobre la naturaleza del incidente, si se produjo o no epistaxis en el momento del trauma y si el paciente se nota alguna deformidad. También es importante interrogar a cerca de la existencia de obstrucción nasal previa o fractura para evitar confusiones con deformidades preexistentes a la hora de realizar una buena exploración.

    Before any trauma to the central part of the face, the possibility of a nasal fracture should be suspected. Apart from the legal connotations and the need to draw up an injury report, a good anamnesis is essential to guide the diagnosis, inquiring about the nature of the incident, whether or not there was epistaxis at the time of the trauma and if the patient notices any deformity.     A good physical examination can reveal most nasal fractures, although evaluation in the first 3-6 hours may be hampered by edema. Signs of a fracture include very painful palpation, deformity with protrusion or subsidence of the bony surfaces, abnormal mobility, bony crepitus, ecchymosis and hematoma. Other signs that would indicate more severity include scleral chemosis, periorbital ecchymosis, subconjunctival hemorrhage and subcutaneous emphysema. The examination should be performed after aspirating the hematic debris from the nostrils and applying a topical nasal mucosal decongestant, so that possible mucosal lacerations and the status of the septum can be detected, assessing for septal dislocation and/or septal hematoma.

    Some authors find the performance of simple X-rays of the patient's own bones to be inefficient and unnecessary.     Deben tratarse primero las partes blandas, resecando todos los tejidos mortificados que no tengan posibilidad de supervivencia (FRIEDRICH), pero deben conservarse todas las porciones cutáneas posibles, aun las de vitalidad dudosa, pues una resección excesiva compromete el resultado funcional y estético. Debe lavarse bien la herida con suero fisiológico para arrastrar pequeños cuerpos extraños que alterarían la calidad de la cicatrización. Antes de efectuar ninguna acción de cierre debe aprovecharse el trayecto de la herida para realizar a su través las maniobras necesarias para la reposición de los fragmentos a cielo abierto y, si es preciso, la inmovilización por sutura. La extracción de cuerpos extraños debe asegurarse antes de cerrar la herida.

    The soft tissues should be treated first, resecting all mortified tissues that have no chance of survival (FRIEDRICH), but all possible cutaneous portions should be preserved, even those of doubtful vitality, because excessive resection compromises the functional and esthetic result. The wound should be washed well with physiological saline to remove small foreign bodies that would alter the quality of healing. The removal of foreign bodies must be ensured before closing the wound.

    6. naso-septal fractures seen three or more weeks after the trauma.

    Fractures produced by an anteroposterior force will show, if there was sufficient energy, a collapse of the bony pyramid.     Si la fractura es conminuta o inestable puede colocarse un “packing” de gasas con antibiótico que servirá como soporte y frenará el sangrado. Además, para mantener la posición y evitar nuevos daños se colocarán steri-strips definiendo la posición de la nariz y una férula rígida de protección. La estabilidad de la fractura debe ser valorada a los 10 días post-reducción, y si la inestabilidad persiste se mantendrán los apósitos de contención externos (steri-strips y férula) otra semana más. Asociar como tratamiento médico (medidas únicas si no hay fractura) lavados con suero fisiológico, frío local, analgésicos y antiinflamatorios.

    The nose is anesthetized and prepared in the same manner as for closed reduction. Many of the cases requiring open reduction are due to impacted cartilage and/or bone fragments. Access to the septum is made through a hemitransfixing incision on the dislocated side. Other accesses to the fracture zone are made through lateral intercartilaginous incisions.

El tratamiento es mediante una incisión horizontal sobre la base del septum a través de la cual se producirá su drenaje, y la introducción de gasa de borde que se mantendrá unos 7 días para evitar su cierre y nueva formación. Además la colocación de un “packing” de gasas en ambas fosas nasales por 2-3 días prevendrá su reacumulación.

    Edema, ecchymosis and epistaxis usually resolve spontaneously. Infection is rare, but antibiotic prophylaxis should be administered in immunocompromised patients and in cases of septal hematoma. CSF rhinorrhea is associated with more severe trauma and is indicative of fracture of the posterior frontal sinus or cribriform plate. Emphysema of the face and neck may also occur although it usually resolves spontaneously.

En las fracturas nasales una leve deformidad después de una reducción cerrada suele ser frecuente, la exacta posición anatómica de los fragmentos puede ser difícil de detectar sólo por palpación, especialmente si hay edema. Si esto supusiese un problema para el paciente, estético o funcional, una rinoplastia reconstructiva sería la solución.

    Residual osteitis is sometimes observed in compound fractures or in fractures associated with infected hematomas. In these cases, good debridement and antibiotherapy are the treatment of choice. In nasal fractures a slight deformity after closed reduction is frequent, the exact anatomical position of the fragments may be difficult to detect by palpation alone, especially if edema is present. If this is a problem for the patient, aesthetic or functional, a reconstructive rhinoplasty would be the solution.
