After creating a campaign, there will be somewhere your Lead Capture Form, a very important part of the Lead Capture Pages, which captures the information of the lead.

You can check the Form section under Tools menu on Click Track Profit, as well as inside CTP Blueprint where I have a complete video tutorial about TrafficWave including complete customization of the Form.
